Can I use my NHS email account to create a Microsoft Form survey?

Yes, you should have access to Microsoft Forms with your NHS account. If you see  "Your organisation has not enabled Microsoft Forms for your current account". Speak to your local IT department who should be able to enable Forms for your account.
